We kindly request that when uploading documents for your application, enrollment packet, or the health office, you do not send photographs (JPEG/HEIC) of the hard copy form. If you will be using your phone to "scan", please follow the instructions outlined HERE.

If you prefer, print documentation and mail or deliver to the SMLS Main Office addressed to Kimberly Tabler, Admissions.
